SOURCE is a Georgia Medicaid waiver that combines primary medical care with long-term, in-home support — for older adults and adults with disabilities who have chronic conditions and would otherwise need nursing-home care.
What makes SOURCE different is the tight coordination: a care coordinator works with your primary care doctor and service agencies to manage your health, prevent hospital stays, and keep you safely at home.

Who qualifies
- Older adults (generally 65+) or disabled adults (18+) who need a nursing-home level of care.
- Has Georgia Medicaid, meeting the program's income and asset limits.
