The Community Care Services Program (CCSP) is a Georgia Medicaid waiver that provides home and community-based services to older adults and people with physical disabilities who would otherwise need nursing-home care but want to stay in their own home.
CCSP helps your family member keep their independence and quality of life — with a case manager who coordinates a personalized plan of care.

Who qualifies
- Older adults (generally 60+) or younger people with physical disabilities who need a nursing-home level of care.
- Has Georgia Medicaid, meeting the program's income and asset limits.
- Prefers to receive care at home or in the community rather than in a facility.
