As a Georgia Medicaid Program, Carenbe’s Structured Family Care provides financial assistance, training and support, and coordination of care services to qualified families.

Become a Paid Caregiver for your Family Member at Home
FINANCIAL ASSISTANCE
If your family member qualifies for services and you qualify as a caregiver, get paid weekly for services through Medicaid.
EXPERT COACHING TEAM
Your health coach understands your unique needs and challenges. They will provide tools and resources to assist you.
COORDINATION OF CARE
Coordinators work with caregivers to develop a care plan for their loved one and to coordinate the delivery of services.
EMOTIONAL SUPPORT
Carenbe cares about your loved one’s well-being and is here to listen and answer any questions you may have.

Who is eligible for Structured Family Care?
Someone who has Medicaid
Is 18 years of age or older
Needs at least 5 hours of help with daily living activities each day
Lives in Georgia and in the same home as the person providing care
Who is eligible to be a Caregiver?
Related by blood, marriage or adoption to the individual needing care (child, cousin, daughter-in-law or son-in-law, aunt, uncle, niece or nephew).
A spouse or legal guardian does not qualify
Is 18 years of age or older

Hi! I’m Dr. Anna Elmers
a physical medicine and rehabilitation specialist, and founder of Carenbe.
As a rehabilitation physician, I have seen the entire continuum of care. My years in the acute inpatient setting have revealed many opportunities for improvement as the injured patient transitions from inpatient to home.
Together with our sister company Desiger Health Partners, we offer the full spectrum of care previously only available to the inpatient. From skilled caregivers to case management to therapy, our goal is to bridge the gap and help patients live the best life possible.
