Carenbe’s skilled nursing team is equipped to handle the medically complex catastrophic client and is here to provide excellent care, including visits for educational services, colostomy care, wound care and hourly nursing services.

Hi! I’m Dr. Anna Elmers

a physical medicine and rehabilitation specialist, and founder of Carenbe.

As a rehabilitation physician, I have seen the entire continuum of care. My years in the acute inpatient setting have revealed many opportunities for improvement as the injured patient transitions from inpatient to home.

Together with our sister company Desiger Health Partners, we offer the full spectrum of care previously only available to the inpatient. From skilled caregivers to case management to therapy, our goal is to bridge the gap and help patients live the best life possible.
